Qualifications:

• Bachelor's degree in Social Work is required.

• Must hold an Ohio License (LSW or LPC).

Hours and Compensation:

• Full-time position (40 hours/week) with a schedule of 8am - 4:30pm and weekend/holiday rotation.

• Hourly pay range of $29-$41, depending on experience.

Responsibilities:

• As part of a multidisciplinary team, the Social Worker Care Manager ensures efficient patient progress across the continuum of care while maintaining high standards of patient care.

• This role involves coordinating discharge planning and intervention for all inpatients, as well as providing support to outpatients as necessary.

• The Care Manager addresses the complex psychosocial needs of patients, offering crisis intervention to both patients and their families, and developing discharge plans for high-risk patient populations.

• The overarching goal is to promote patient wellness, reduce readmission rates, and improve overall patient care outcomes.
