This role requires a Licensed Therapist who will be responsible for providing effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources. The individual in this position will assess patients for transition needs, conduct complex psycho-social assessments and interventions, and promote timely discharge and prevent readmissions. This role also includes precepting new staff members, participating in quality improvement initiatives, and providing education to physicians, patients, families, and caregivers.
QUALIFICATIONS:
- Required qualifications include licensure as a Clinical Social Worker in the state of California.
- Preferred qualifications include accreditation as a Case Manager and a minimum of two years of acute hospital experience. Other required skills include organizational skills, excellent communication skills, ability to lead and coordinate activities in a fast-paced environment, critical thinking and problem-solving skills, and computer literacy.
Job Type: Full-time
Salary: $85,000.00 - $120,000.00 per year
