We are seeking a skilled Occupational Therapist who is passionate about delivering top-quality Occupational Therapy services.

  • Key Responsibilities:

    1. Conduct assessments, diagnostic tests, skilled treatments, and continuous evaluations on clients under a medically approved Plan of Care.
      • a. Timely completion of assessments in accordance with Agency policy.
      • b. Design treatment plans and establish goals for improved function.
      • c. Provide assessment findings and goals to contribute to the client care plan and obtain necessary physician orders.
      • d. Thoroughly document all findings, plans, interventions, and client progress (outcomes) on designated forms in adherence to Agency guidelines.
      • e. Communicate plans and changes to physicians, nursing Case Managers, and other Agency staff through the care plan, written progress notes, and participation in care conferences.
    2. Guide and instruct clients in therapeutic self-care and creative activities aimed at enhancing independence and physical and cognitive functioning.
    3. Advise, consult, and educate clients, family members, and Agency staff on appropriate treatment procedures and supportive activities.
    4. Create and/or order suitable adaptive/assistive devices and/or splints to promote optimal functioning.
    5. Instruct clients and their families in the specific program designed for their needs.
    6. Perform duties in compliance with Agency policies and procedures, as well as professional and community standards.
      • a. Provide services in accordance with applicable state, federal, and JCAHO standards.
      • b. Attend meetings and required educational programs.
      • c. Take personal and professional responsibility for development and identify learning/developmental needs to supervisor.
    7. Attend care conferences, make interdisciplinary referrals, and participate in care coordination as necessary.
    8. Participate in the Clinical Record Review Committee.
    9. Perform other related duties and responsibilities as needed.


  • Graduate of an accredited occupational therapy curriculum, accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association; or eligible for the National Registration Examination of the American Occupational Therapy Association.
  • Hold a current registration or license as required by the state(s) of practice.
  • Minimum of two (2) years of experience in a healthcare facility or equivalent.
  • CPR certification.
  • Demonstrate excellent verbal and written communication skills, as well as good interpersonal skills.
  • Hold a valid driver's license and have an insured automobile that meets state/Agency requirements. Provide evidence of a safe driving record and reliable transportation.
