Temp Physical Therapist

Location: Santa Monica, CA

Our company is currently seeking a Temporary Physical Therapist to join our team in Santa Monica, CA!

In this role, the Physical Therapist will assess and treat residents under the guidance of a physician. You will utilize various therapeutic procedures to restore muscle strength, coordination, and body alignment, as well as relieve pain and improve movement and function. As a member of our interdisciplinary team, you will also provide support and information within the field of Physical Therapy.


  1. Review available resident information and evaluate its impact on the assessment and treatment process.
  2. Conduct and document thorough functional assessments of residents, including assessments of home/vocational and equipment needs.
  3. Create and implement individualized treatment plans based on resident needs and achievable functional goals.
  4. Ensure compliance with evaluation, treatment, and documentation guidelines, maintaining timely and accurate department records.
  5. Instruct Assistants in treatment procedures and provide close supervision when they are working with residents.
  6. Enter a progress note in the resident record following each treatment session.
  7. Revise treatment plans as necessary throughout the process.
  8. Document the course of treatment in a clear and concise discharge summary.
  9. Orient residents to program services and inform them of any potential risks during procedures.
  10. Work with nursing staff, residents, and families to maximize functional abilities.
  11. Instruct residents, family members, or caregivers about exercise programs and therapeutic procedures for continued treatment at home.
  12. Assist in the referral process for residents requiring additional services after discharge, and maintain knowledge of community resources.
  13. Participate in rehabilitation team conferences to discuss resident progress, problems, or needs.
  14. Attend other department and facility meetings as required.
  15. Access continuing education opportunities to maintain clinical competency.
  16. Participate in the quality improvement process.
  17. Promote the programs and services of Heritage Healthcare through interactions with the community.
  18. Adhere to regulatory and professional standards and guidelines.
  19. Provide feedback on the clinical performance of Assistants to the Rehab Manager.
  20. Adhere to facility policies and participate in quality improvement and safety programs.
  21. Perform other duties as assigned to provide quality care to residents.
  22. Demonstrate knowledge of reimbursement guidelines and allocate therapy resources accordingly.
  23. Understand OBRA regulations, HIPAA, Local Coverage Determination, and third party reimbursement guidelines.
  24. Support the company’s commitment to compassionate care and maintain a professional image within the Rehab Program.
  25. Perform duties independently, recognizing when direct supervision is needed, and willing to assist others.
  26. Maintain expected productivity level as discussed and expected for your position at your particular facility/facilities.

  1. Bachelor's or Master's degree in Physical Therapy.
  2. Current license in the state of practice.
  3. Prefer supervision, training, and/or experience in a similar setting.
  4. Certification, if applicable, and demonstrated competency in specialized areas.
  5. Excellent oral and written communication and interpersonal skills.
  6. Demonstrated competency in the principles, methods, materials, and equipment used in physical therapy.
  7. Ability to assess resident needs and develop comprehensive care plans.
  8. Knowledge of accreditation standards and compliance requirements.
  9. Ability to perform physical therapy activities safely and for prolonged periods.
