Qualifications:
• Must possess a Bachelor's degree or higher in Science in Allied Health Professions in Physical Therapy from an accredited program
• Current license in Physical Therapy in the State of Ohio required
• Preferred two years of community/home health experience as a physical therapist
• Must possess a current CPR (AHA BLS) certificate
• Valid Ohio driver's license and active motor vehicle insurance

Hours and Compensation:
• Full-time position with hours from 8am to 4:30pm
• Salary range between $70K to $102K, depending on experience
• Full benefit package including a $25K sign-on bonus

Responsibilities:
• Provide physical therapy services to home health patients
• Supervise Physical Therapy Assistants
• Stay knowledgeable about the standard of practice of Physical Therapy in a home health setting
• Assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care
• Evaluate the home environment
• Assess muscle strength, mobility, gait, and range of motion for potential rehabilitation
• Direct physical therapy treatment
• Instruct patients and families/caregivers on the use and care of therapeutic appliances
• Determine priority needs for physical therapy
• Report patient's reaction to treatment or changes in condition to the physician
• Complete medication reconciliation for therapy-only patients as needed
• Complete OASIS charting within three days
• Initiate physical therapy program and provide instructions to other personnel and/or family/caregiver members involved in the patient's care
• Chart each visit within 72 hours
• May train patients in the use of prosthetic devices
• Identify patient and family/caregiver needs for other home health services and provide necessary referrals
• Prepare and submit clinical and progress summaries based on the attainment of goals
• Participate in discharge planning
