PT Assistant or Rehabilitation Therapist

Physical Therapist (PT) Per Diem

We are currently seeking a Physical Therapist to join our team in Redondo Beach, CA! As a Physical Therapist, you will play a crucial role in helping patients with injuries and chronic health conditions regain their range of motion, manage their pain, and improve their quality of life. Using a variety of techniques such as hands-on therapy, exercises, electrical stimulation, ultrasound, and more, you will evaluate and treat residents under orders from a physician. This includes restoring muscle strength, coordination, body alignment, relieving pain, and improving movement and function. As a member of the interdisciplinary team, you will provide support and information within the area of Physical Therapy while adhering to facility policies and procedures.

Responsibilities:

  1. Review available resident information and evaluate its impact on the assessment and treatment process.
  2. Conduct and document a thorough functional assessment of the resident, including home/vocational and equipment needs.
  3. Develop and implement an individualized treatment plan based on resident needs and achievable functional goals.
  4. Comply with evaluation, treatment, and documentation guidelines while maintaining timely and accurate records.
  5. Instruct Assistants in treatment procedures and provide close supervision.
  6. Enter a progress note in the resident record after each treatment session.
  7. Revise treatment plans as needed throughout the treatment process.
  8. Document the course of treatment in a clear and concise discharge summary.
  9. Orient residents to program services and inform them of any potential risks during procedures.
  10. Work with nursing staff, residents, and families to maximize resident's functional abilities.
  11. Instruct residents and family/caregivers on exercise programs and therapeutic procedures for continued use at home.
  12. Assist in the referral process for additional services.
  13. Attend and participate in rehabilitation team conferences and other department/facility meetings.
  14. Access continuing education opportunities to maintain clinical competency.
  15. Participate in the quality improvement process.
  16. Promote the programs and services of Heritage Healthcare within the community.
  17. Adhere to regulatory and professional standards and guidelines.
  18. Provide input on the clinical performance of assistants, as appropriate.
  19. Adhere to facility policies and procedures and participate in quality improvement and safety programs.
  20. Perform other assigned duties to provide quality care to residents.
  21. Understand reimbursement guidelines and allocate therapy resources efficiently.
  22. Demonstrate knowledge of regulations and guidelines in the field.
  23. Support the company's commitment to Compassionate Care and maintain a professional image.
  24. Perform duties in an independent manner and provide assistance when needed.
  25. Maintain expected productivity levels.

Qualifications:

  1. Bachelor/Masters of Science degree in Physical Therapy.
  2. Current license in the state of practice.
  3. Prefer supervision, training, and/or experience in a similar setting.
  4. Certification in relevant areas of specialization, if applicable.
  5. Excellent oral and written communication and interpersonal skills.
  6. Demonstrated competency in physical therapy principles, methods, materials, and equipment.
  7. Ability to assess resident needs and develop comprehensive care plans.
  8. Knowledge of accreditation standards and compliance requirements.
  9. Ability to perform physical tasks safely and for prolonged periods.
