The Primary Care Physician is responsible for conducting comprehensive assessments and collaborating with the team to create individualized care plans for each participant. Under the direction of the Medical Director, the Primary Care Physician oversees the care of participants in nursing homes and hospitals, including admissions, discharges, and the coordination of medical specialists. Additionally, the Primary Care Physician provides guidance and support to Nurse Practitioners and other team members.
Essential Job Duties:
- Conduct comprehensive assessments and provide primary care to participants
- Collaborate with community-based physicians and the Interdisciplinary Team (IDT) for discharge planning
- Participate actively in the IDT, communicating participant changes and contributing to care planning decisions
- Maintain regular visits to nursing homes to provide ongoing care to participants
- Share in the responsibility of night and weekend calls

- Interview, examine, and treat clinical patients
- Educate patients and families on wellness, prevention, and early detection
- Make referrals based on examination and patient needs
- Participate in quality improvement, management, and continuing education programs
- Assist in resolving patient complaints, requests, and inquiries
- Maintain patient confidentiality according to federal guidelines
- Complete daily medical records documentation
- Cover night calls as necessary
Job Requirements:
- Completion of an accredited residency program approved by the Accreditation Council for Graduate Medical Education
- Doctorate Degree
- Board Certification in Internal Medicine, Family Practice, or Geriatrics (preferred)
- Active California Physician License without restrictions
- Active state CDS Certification and DEA
- Must be currently enrolled or eligible for enrollment to provide services to Medicare and Medi-Cal participants
