As an RN Care Manager, you will assume an exciting hybrid role, balancing in-person visits to homes with the convenience of remote coaching. Your objective is to support family caregivers and their loved ones by providing personalized care and guidance. No overnight shifts, weekends, or on-call responsibilities, ensuring a perfect work-life balance.
Primary Duties:
- Perform home visits in the assigned area to evaluate safety, monitor health changes, and provide clinical coaching, including medication reviews.
- Follow an established care plan and offer ongoing coaching through our digital app, providing resource referrals and advice based on best practices.
- Establish trusting relationships with caregivers to promote positive changes and help reduce burnout.
- Maintain accurate and timely documentation, ensuring compliance with standards and regulations.
- Take on additional projects and responsibilities as required.
Qualifications:
- An active Registered Nurse (RN) License in the state of employment.
- At least two years of recent experience in direct care for older adults or individuals with disabilities.
- Ability to travel within the assigned area (a valid driver's license and auto insurance are essential).
- Experience in case management, care planning, and assessment, particularly in eldercare, disabilities, or complex medical needs.
- Proficiency with common software and web-based applications, including MS Office and case management systems.
- Bilingual skills are a plus and may impact compensation.
