Registered Nurse Case Manager

As an RN Case Manager, you will have a unique role that includes in-person home visits as well as remote coaching. Your main goal will be to provide personalized care and guidance to family caregivers and their loved ones. The best part? Responsibilities include:

  • Conducting home visits in your assigned area to assess safety, monitor health changes, and provide clinical coaching, including medication reviews.
  • Following a prescribed care plan and offering ongoing coaching through our digital app, which includes resource referrals and best practice advice.
  • Building trusting relationships with caregivers to promote positive changes and reduce burnout.
  • Maintaining accurate and timely documentation, ensuring compliance with standards and regulations.
  • Assuming additional projects and duties as required.

Requirements:

  • An active Registered Nurse (RN) License in the state of Massachusetts.
  • At least two years of recent experience in direct care for older adults or individuals with disabilities.
  • Capability to travel within the assigned area (a valid driver's license and auto insurance are mandatory).
  • Experience in case management, care planning, and assessment, particularly in eldercare, disabilities, or complex medical needs.
  • Proficiency in common software and web-based applications, including MS Office and case management systems.
  • Bilingual skills are desirable and can impact compensation.
