RN Case Manager

As an RN Case Manager, you will have a unique role that combines in-person home visits with the convenience of remote coaching. Your mission is to provide personalized care and guidance to family caregivers and their loved ones. No night shifts, weekends, or on-call duties, ensuring a perfect work-life balance for you.
Essential Duties and Responsibilities:
- Conduct home visits in the designated area to evaluate safety, monitor health changes, and provide clinical coaching, including medication reviews.
- Adhere to an established care plan and offer ongoing coaching through our digital app, providing resource referrals and offering best practice advice.
- Establish trusting relationships with caregivers to foster positive changes and mitigate burnout.
- Maintain accurate and timely documentation to ensure compliance with regulations and standards.
- Assume additional projects and responsibilities as required.
Qualifications:
- Active Registered Nurse (RN) License in Massachusetts.
- Minimum of two years of recent experience in direct care for older individuals or adults with disabilities.
- Ability to travel within the designated area (valid driver's license and auto insurance required).
- Experience in case management, care planning, and assessment, particularly in eldercare, disabilities, or complex medical needs.
- Proficiency in common software and web-based applications, including MS Office and case management systems.
- Bilingual skills are advantageous and can impact compensation.
