Nursing Care Coordinator

As an RN Care Manager, you will have the opportunity to work in a hybrid position, combining in-person home visits with the convenience of remote coaching. Your goal will be to provide personalized care and guidance to family caregivers and their loved ones. Best of all, there are no night shifts, weekends, or on-call duties, ensuring a perfect work-life balance.
Key Responsibilities:
- Conduct home visits in your assigned area to assess safety, monitor health changes, and provide clinical coaching, including medication reviews.
- Follow an established care plan and offer ongoing coaching through our digital app, referring resources and providing best practice advice.
- Develop trusting relationships with caregivers to promote positive changes and help reduce burnout.
- Maintain accurate and timely documentation, ensuring compliance with standards and regulations.
- Take on additional projects and duties as necessary.
What You Bring:
- An active RN license in the state of employment.
- At least two years of recent experience in direct care for the elderly or adults with disabilities.
- Ability to travel within your assigned area (a valid driver's license and auto insurance are required).
- Experience in case management, care planning, and assessment, particularly in eldercare, disabilities, or complex medical needs.
- Proficiency with common software and web-based applications, including MS Office and case management systems.
- Bilingual skills are a plus and may impact compensation.
